Registered Nurse (RN) – Full-time and part time 3rd shift on LTC and full time 2nd shift in Short Term Rehab. Must be licensed to practice in the Commonwealth of PA with at least one-year experience. Responsible for resident care and ensuring residents receive care outlined in their care plan.
Licensed Practical Nurse (LPN) – Full-time 2nd and 3rd shift. Must be licensed to practice in the Commonwealth of PA with at least one-year experience. Responsible for resident care and ensuring residents receive care outlined in their care plan.
Certified Nurse Aide (CNA) – Full-time 1st shift and Part-time all shifts available. Valid Nurse Aide Registration in the Commonwealth of Pennsylvania is required with at least one year of experience. Responsibilities include compassionate care while building long term relationships and assisting patients and residents with vital elements of their health and wellbeing.
Director of Nursing – The DON is an exempt position responsible for the overall quality clinical outcomes of the facility. Requires working collaboratively with the interdisciplinary teams and providing direct supervision to the Nursing Department team. Requires excellent organizational/communication skills, clinical knowledge, and leadership skills. Main responsibilities include- Assuring compliance with DOH, CMS, collaborating with NHA and remaining accountable for the operational and financial performance of the department, driving quality initiatives, and ensuring QAPI/Infection Control programs are followed, leading efforts for staffing and retention of Nursing, communicating timely/accurately with employees and customers, meeting with staff/management regularly to ensure initiatives and goals are being met.
Personal Care Aide (PCA) – Part time (3:00pm to 11:00pm and 11:00pm-7:00am) available within Grace Mansion PCH. High school diploma or GED required, PCA experience with older adults preferred. Ideal direct care experience for nursing students. The PCA provides routine assistance with the activities of daily living in accordance with Federal, State, and Local standards, and established policies and procedures to assure that the highest degree of resident care is maintained.
Life Enrichment Assistant – (1) Full-time position (9:00am-5:00pm) and (1) Part-time position (9:00am-5:00pm) is currently available in our Life Enrichment Department. This position requires every other weekend and every other holiday. Experience with senior citizens/older adults a plus. Must have passion to work with seniors and engage them in a variety of activities, including daily mass, dining assistance, therapy animals, and a host of daily/weekly activities!
