Registered Nurse (RN) – Full time 2nd and 3rd shifts, part time all shifts. Must be licensed to practice in the Commonwealth of PA with at least one-year experience. Responsible for resident care and ensuring residents receive care outlined in their care plan.
Licensed Practical Nurse (LPN) – Full-time 3rd shift and part-time 2nd and 3rd shift available. Must be licensed to practice in the Commonwealth of PA with at least one-year experience. Responsible for resident care and ensuring residents receive care outlined in their care plan.
Certified Nurse Aide (CNA) – Full-time 1st shift and part-time all shifts available. Valid Nurse Aide Registration in the Commonwealth of Pennsylvania is required with at least one year of experience. Responsibilities include compassionate care while building long term relationships and assisting patients and residents with vital elements of their health and wellbeing.
Admissions Nurse – (1) Full-time, 1:00 pm -9:00 pm (37.5 hrs/wk M–F/No holidays or weekends.) licensed RN for our Short-Term Rehab unit. Must have 2+ years of experience overseeing admissions in a hospital or long-term care facility. The Admissions Nurse will initiate the initial plan of care with authorization of the attending physician and medical director and complete the admission process according to facility policy. This includes explaining STR services to patients and family members and ensuring all patient’s initial needs are met. Communicate with the Inter-Disciplinary team members regarding initial plan of care, patient’s condition, and other essential information.
Rehab Unit RN Supervisor – (2) Full -time positions 7:00am-3:00pm, 3:00-11:00pm. for fast paced sub-acute unit. Full-time, Mon – Fri. and every other holiday. RN with minimum of 5 years LTC/STR experience, including 3+ years in a supervisory role. High-energy, compassionate individual who is responsible for assuring physicians’ orders are followed and quality care is provided. Strong organizational and leadership skills needed. This position requires 75% direct patient care and 25% supervisory and/or administrative/non-direct care duties.
Assistant Director of Nursing – As ADON, you will be responsible for assisting with management of overall quality clinical outcomes and education for the facility. The ADON works collaboratively with the Director of Nursing (DON), interdisciplinary teams and assists with supervisory and leadership tasks. Priority one will always be quality resident care and employee relations. Requires excellent organizational and communication skills, clinical knowledge, and leadership skills. Assures compliance with DOH, CMS and other regulatory and/or accrediting bodies, assists with staffing and retention of Nursing team, and meets with staff and management regularly to ensure clinical and education initiatives and goals are met. Minimum three years nursing experience, Health Administration BS or MS. Two or more years in ADON/supervisory/management position, with staff development and training program experience. Knowledge of state and federal regulations pertaining to LTC facility required. Must be a Licensed Registered Nurse within the Commonwealth of Pennsylvania.
Wound Care Nurse – Part time position with flexible hours. The Wound Care Nurse is responsible for assessing skins alterations, developing plans of care, implementing orders and treatments, and evaluating the progress of specialized skin and wound conditions. The ideal applicant will be skilled in identifying wound types, analyzing trends, and supervising nursing personnel to deliver nursing care within his/her scope of practice. Must be professional, PA State licensed RN, have wound certification and current CPR certification. Prior wound care experience is required.
Assistant Personal Care Home Adminstrator – Full time (37.5 hours/week) Monday-Friday. The Assistant Administrator assists the Administrator in the day-to-day operations of the community. This position aids with supervisory functions of all personal care and medication staff in accordance with applicable Federal, State, and Local standards and established policies and procedures to assure that the highest degree of resident care is always maintained. The ideal candidate for this position will have a valid PA State Licensure as an LPN for 3 years, have 2 or more years supervisory experience in a PCH as an LPN and have experience with DHS PA Chapter 2600 regulations.
Maintenance/Security – Part-time, (15 hours/week) 11:00pm-7:00am, every other weekend required. Monitor and patrol assigned areas to provide a safe and secure environment for residents, guests, and employees by anticipating potential problems and react to disturbances and prohibited conduct. Minimize opportunities for loss or damage and respond to emergency situations as needed. HS diploma/GED required, prior facility maintenance and/or security experience preferred.
Regional Maintenance Technician – Bethlehem (1) Full time (37.5 hours/week) Monday-Friday 8:00am to 4:00pm Responsible for maintaining assigned facility/facilities (Holy Family Apartments, Antonian Towers and independent market CCRC) in a clean, sanitary, safe condition; completing preventative maintenance, general repair, grounds keeping and interior cleaning. Perform other related duties as directed by the Property Manager or the Director of Housing.
Custodian-Berks County – (1) Part Time (24 hours/week) Monday, Wednesday, Friday Custodian needed for senior apartment buildings in Berks County. Duties include washing windows, walls, floors, vacuuming, dusting, cleaning bathrooms and common areas, painting walls and some seasonal grounds maintenance. Valid PA driver’s license and the ability to respond to emergency calls on a rotating schedule including evenings and weekends, is required. Prior facility janitorial/custodian experience required.
