Open Positions

RN House Supervisor – Full-time (11pm-7am). Qualifications: Licensed RN (Commonwealth of PA), 3+ years of nurse supervisory experience, preferably in skilled nursing or related setting. The RN House Supervisor has clinical and supervisory oversight on their scheduled shift. Primary responsibilities include review of staffing at the start of each shift to maintain quality resident and patient care, completing daily rounds, monitoring admissions and discharges, and ensuring appropriate documentation. This key position assists in the orientation of new staff and supervises/mentors team members.

Assistant Unit Supervisor – Full-time (7am-3pm). PA licensed RN with at least 3 years’ experience on a Med/Surge LTC/STR or hospital unit. 2+ years’ experience in a nursing supervisor role required. Assists with management and oversight of the Med/Surge (rehab) Unit including quality patient care, staff accountability and operational effectiveness.

Admissions Coordinator – (Full – Time) Complete all pre-admission and admission forms accurately. Obtain required resident, responsible party, and staff signatures on all admission forms as designated by department director. Follow-up on inquiries by conducting tours of the facility, answering phone calls, and providing families, vendors and potential residents with brochures and information. Perform assigned administrative duties in an efficient manner, in accordance with established procedures to assure that a successful, viable, Admissions process is always maintained.

Assistant Unit Supervisor – Full-time (7am-3pm). PA licensed RN with at least 3 years’ experience on a Med/Surge LTC/STR or hospital unit. 2+ years’ experience in a nursing supervisor role required. Assists with management and oversight of the Med/Surge (rehab) Unit including quality patient care, staff accountability and operational effectiveness.

Registered Nurse (RN) – Full-time and Part- time (All shifts). PA licensed RN with 1+ years’ experience.
Responsible for recording vital signs, administering medications and treatments.
monitoring resident/patient status; electronic medical record documentation in
accordance with care plan, policy, and procedures.

Licensed Practical Nurse (LPN) – Full-time and Part-time. PA licensed LPN with 1+ years’ experience.
Responsible for administering medication, treatments, and quality resident/patient
care as outlined in care plan and in accordance with policy and procedures.

Certified Nurse Aide (CNA) – Full-time and Part-time. Valid PA Nurse Aide Registration with 1 year of
experience. Responsible for compassionate care and assistance with activities of
daily living. CNAs answer call lights and determine how best to assist patients and
residents so they receive comprehensive and personalized care.

Clinical Educator/ADON – Full time. Oversee the entire HFSL Education and Training Program which includes maintaining documentation and recordkeeping of mandatory and non-mandatory in-services and training, including Relias and other facility training programs. Assist the DON in maintaining a highly successful, efficient, and effective Nursing Team. Excellent verbal and written communication skills and outstanding presentation and demonstration skills required. Leadership and management experience. Knowledge of nursing and medical practices and procedures, as well as laws, regulations pertaining to nursing home operations. Registered Nurse licensed in the Commonwealth of PA. Minimum five years nursing experience, Health Administration, BS, or MS. 3+ years in supervisory/management position, with staff development and training program experience. Computer proficiency and knowledge of Point-Click-Care (PCC) required.

Personal Care Aide / Medication Tech (PCA/MT) – Part-time (11pm-7am). PCA or caregiver experience preferred, but not required. Compassionate people and nursing students welcome! Assists residents with activities of daily living in accordance with regulations, policies, and procedures to ensure the highest degree of comprehensive, personalized and quality resident care is maintained at all times. 

Life Enrichment Assistant – Full-time and Part-time. Assist with the planning and implementation of resident activities to encourage socialization and provide relaxation, fulfillment, and entertainment and to improve daily living skills. Activities including daily mass, dining assistance, therapy animals and many more daily, weekly, and seasonal activities! Experience with older adults preferred, but not required if you have the enthusiasm and energy to work with seniors!

Medical Secretary/Health Unit Coordinator Full time.  This position coordinates all administrative duties on the high-volume, fast-paced short-term rehab unit and aids the Unit Supervisor with the day-to-day clerical tasks, communications, and organization of the unit. 1-2 years prior medical secretary/health unit coordinator experience; knowledge of medical terminology required. Computer proficiency required; Point Click Care (PCC) experience a plus! Excellent customer service and organizational skills. Exceptional written/verbal communication and interpersonal skills are a must.

Resident Concierge – Full-time (37.5 hours/week) with the choice of a 7:00 am – 3:00 pm OR 10:00 am – 6:00 pm schedule. Every other weekend and holiday is required. Responsible for providing the residents and patients reassurance and comfort to ensure the highest degree of satisfaction. Previous experience in healthcare is not required, but prior caregiving or housekeeping experience is a definite plus and the ability to provide quality customer service.
