Setting the Standard for Heart Failure Recovery in Skilled Nursing Care

Holy Family Senior Living is proud to announce that we have been certified as an American Heart Association (AHA) Skilled Nursing Facility (SNF). This certification means that Holy Family has demonstrated adherence to AHA’s evidence-based guidelines for heart failure or stroke care, ensuring high-quality care and a smooth transition to the community. This certification signifies a commitment to standardized care practices, program management, patient education, and care coordination, ultimately improving patient outcomes and building confidence in the facility.

The overarching goal of the Certification encompasses the patient and provider, enhancing the relationship with local hospitals, and engagement with insurers and payors. The certification is focused on standardizing heart failure patient care and is designed to evaluate each skilled nursing facility against a professional set of criteria based on demonstrated adherence to key standards and a rigorous review process.

The AHA Program’s Goals:

  • Ensures evidence-based heart failure guidelines are driving decision making at all levels of care represented in the skilled nursing facility.
  • Provides guidance and leadership on measurably improving care coordination and communication between and among care providers.
  • Demonstrates that high levels of compliance for one or more of these entities through certification or recognition results in measurable improvements in clinical quality of care and cost effectiveness.
